Levi Car Inspection Form

Institute for Automotive Service Excellence (ASE) or American Advanced Technicians Institute (AATI) or by a mechanic which is under the direct supervision of a certified AATI or ASE mechanic. We provide a form you can download and print to take to a certified mechanics/inspector to complete. Mechanics/inspectors will not have these forms at hand, you are required to take your states form to your appointment for completion. Please print the form for the state you will be driving in, fill out the section designated for you and provide the form to the inspector/mechanic to complete. It may be a good idea to print multiple forms in the event your form is filled out incorrectly or stained. Inspections are good for a year and must be renewed every year. 

Submit completed passing inspection form with all other required documents at the time you complete your application to drive with Levi. You can apply at our website https://way.leviapp.com or through your smart phone once you have downloaded the Levi app. Inspections are required and should be done before you have submitted an application to become a driver. If the inspection form is not submitted with your driver application, it will cause further delay and background checks will not commence until we have a complete drivers application package. The following states provide annual inspections and Levi accept these official inspections. Drivers must provide the inspection form from a state licensed facility or TLC’s Safety & Emissions Facility, that is provided with the inspection. Documentation will be verified to insure the validity of the inspection.
