RIDING MORE AFFORDABLY INITIATIVE REGISTRATION

Riding More Affordably Initiative is a plan developed by Levi's CEO to help those less fortunate to obtain door-side transportation. This is particularly beneficial for elderly who may have trouble getting around and customers who live in rural areas and are not in walking distance of a public bus line. Levi understands these problems and wants to help! Once you have been signed up with Levi for six months and have had consistent rides during this period, you will be eligible to register for a chance to have a free ride to work or to a doctors appointment. Registration is easy! Complete the registration form below and submit it us. You will be required to email proof of your doctors appointment (provided by appointment card or notice of appointment from you doctors office on letterhead) or proof of employment (can be a current stub or a proof of employment letter from your  supervisor on company letterhead with a contact number) so Levi can valid your request. If Levi is unable to validate your request, your free ride will be denied. Levi wants to help supply a ride but only to those who truly have a real need. A small price to pay for a free ride. Once your package is complete, Levi's representatives will be in touch to notify you of your request status. All registration forms must be submitted three weeks prior to the date of service to allow time for processing. No expectations will be made.
